<p>What is this picture?</p>



<p>Below is an <a href="https://www.sussexfootcentre.co.uk/diagnostic-ultrasound/" target="_blank" rel="noreferrer noopener" aria-label="ultrasound (opens in a new tab)">ultrasound</a> of a ladies little toe.</p>



<p>Many people think that an X-ray is needed to show a break in the bone but this isn’t the case. The picture below is proof. It shows there is a break and that it hasn’t healed.</p>



<p>Sometimes just knowing what has happened is all that is needed to ease a patient&#8217;s worry. Which was all that was needed for this patient!</p>



<p>The break is the sharp spike halfway along the phalanx (phalanx is the toe bone). The arrow marks the spot.</p>

<p>The foot is a hugely complicated design, with the intricate bones, joints, ligaments, and tendons that not only make up the foot and ankle but also the lower limb. It’s the intricacies of the structure of ‘the foot’, that is the reason why I have two degrees on ‘the foot’ both a BSc and an MSc solely focussed on the foot with 12 years experience of doing nothing else that ‘feet’.</p>



<p>During this time I have realised I cannot do my job without imaging and the use of both ultrasound and X-Ray. These in combination with a clinical examination make for a thorough clinical assessment.</p>

<p>Plantar fasciitis is the most common condition patients bring to the clinic. It can cause pain anywhere from soreness when waking up first thing in the morning to debilitating pain that prevents people from going into work.</p>
<p></p>
<p>Now as we have discussed in <a rel="noreferrer noopener" aria-label="previous blog posts (opens in a new tab)" href="https://www.sussexfootcentre.co.uk/blog/plantar-fasciitis-a-brief-overview/" target="_blank">previous blog posts</a>, the plantar fascia should be measuring no more than 4mm in thickness. The patient in question measured 1cm 4mm. Now, this might not sound like much but within the intricate foot architecture, a 1cm difference is massive!</p>
<p></p>
<p>On Saturday 10th August I carried out a steroid injection under <a rel="noreferrer noopener" aria-label="ultrasound guidance (opens in a new tab)" href="https://www.sussexfootcentre.co.uk/blog/x-ray-vs-ultrasound-for-diagnosing-pain/" target="_blank">ultrasound guidance</a> and most importantly under anesthetic, and therefore was completely pain-free. This technique of carrying out the plantar fascia injection under anaesthetic is something that I have been doing for the last few years and it makes for a much better patient experience.<br />Please try not to listen to people saying how painful a heel injection is, Sussex Foot Centre&#8217;s are completely pain-free.</p>

<p>Where was I?</p>

<p>Back to the point&#8230; I carried out the injection on the 10th August and now the patient is pain-free based on her review!</p>

<p>Most people consider a break to be seen on an X-Ray, or so you would think.</p>

<p>In fact, the x-ray showed below looking at the base of the 5th metatarsal was actually missed. Yes, you heard right missed! Which lead the young man to come and see me in the clinic. </p>

<p>Both the young man and his mother presumed it was soft tissue that was the issue, but in fact, this wasn’t the case and the fracture as can be seen with the break in the ‘white line’ on ultrasound demonstrates the usefulness, and quick nature to diagnose a stress fracture.</p>

<p>As we all know the NHS is somewhat overworked, and so we can excuse them for overlooking this, this is where we come in. </p>

<h3>Ingrown toenails are a patient issue we see regularly&#8230;</h3>

<p><!-- divi:paragraph -->We always try to treat our patients&#8217; ingrown toenails with <a rel="noreferrer noopener" aria-label="chiropody (opens in a new tab)" href="https://www.sussexfootcentre.co.uk/services/podiatry/nail-surgery/" target="_blank">chiropody</a>, which we define as cutting and trimming down the side to alleviate the nail into the skin. However, in this poor girls instance, we couldn’t and we instead had to remove the portion of the nail under anaesthetic before putting a special chemical on there to stop it from growing back.</p>

<h4>So What Is Dry Needling?</h4>
<p><!-- /divi:heading --></p>
<p><!-- divi:paragraph --><a rel="noopener noreferrer" aria-label="Dry needling (opens in a new tab)" href="/conditions-treated/verrucae-wart-treatment/" target="_blank">Dry needling</a> is carried out painlessly under local anaesthetic. A sterile needle is used to cause a localised trauma to the verruca itself. The aim of this is to implant the virally infected cells deeper into the tissue where it is quickly detected by the immune system and an immune response is stimulated. This process has been around for nearly 40 years but it&#8217;s only recently reaching the shores of the UK from the United States. It is very effective and suitable for most verrucas.</p>

<h2 class="wp-block-heading"> I am often asked &#8216;what is the most common condition you see?&#8217;</h2>



<p>To be honest it has to be <a href="https://www.sussexfootcentre.co.uk/conditions-treated/chronic-plantar-heel-pain/">plantar fasciitis</a>, of which we have spoken about many times before. The second most common condition that graces Sussex Foot Centre is pain and swelling focal to the big toe joint. This joint is called the 1st, <g class="gr_ gr_7 gr-alert gr_spell gr_inline_cards gr_disable_anim_appear ContextualSpelling ins-del multiReplace" id="7" data-gr-id="7">metatartoso</g>-phalangeal joint. It sounds fancy but it isn’t! It&#8217;s the joint between the metatarsal and the phalanx, which put even simpler, is the toe. This joint takes a vast amount of load and if the foot is functioning properly can take three times our body weight through the joint. </p>



<p>There are many reasons why patients get pain and swelling on the big toe joint but many of them are still theories. These theories are heavily disputed and argued within the literature with no-one taking a prominent position. </p>



<p>The picture below shows the long bone as the 1st metatarsal and the phalanx, with a ‘black blob’ in the middle, this blackness is swelling and therefore inflammatory fluid. It is this that comes as a result of overload to the joint. In this example, the joint became overloaded as a result of the patient training for the London marathon.</p>

<p>But how do we treat it?</p>



<p>The best way to treat this condition is a good <a href="https://www.sussexfootcentre.co.uk/services/orthotics-insoles/">orthotic</a>, in <a href="https://www.sussexfootcentre.co.uk/services/wide-fitting-comfort-shoes/">a good shoe</a>. The idea is to take pressure away from the joint, this will be tried for a good few weeks. Should this fail, then a small injection of cortisone works very well.</p>

<p>Hence why the profession of <a href="https://www.sussexfootcentre.co.uk/services/chiropody/">chiropody</a>, now more commonly known as <a href="https://www.sussexfootcentre.co.uk/services/podiatry/">Podiatry</a> has been around in one format or another for hundreds of years. </p>



<p>The images I have displayed to coincide with this blog post are to show just how useful <a href="https://www.sussexfootcentre.co.uk/diagnostic-ultrasound/" target="_blank" rel="noreferrer noopener" aria-label="ultrasound (opens in a new tab)">ultrasound</a> is as an imaging technique. </p>



<p>X-ray is used as a mainstay imaging modality by GP&#8217;s to &#8216;see what&#8217;s going on&#8217;, an X-ray can be hugely beneficial to look within the joints, to see arthritis, signs of gout and fractures to name just a few. But, what X-ray isn&#8217;t very good at is looking at swelling. </p>



<p>This is where ultrasound really comes into its own! </p>



<p>Ultrasound works well as your Podiatrist can easily and quickly compare one foot to the other, which helps with diagnosis. </p>



<p>The images below show how a patient&#8217;s left big toe joint is normal, but the right foot big toe joint isn&#8217;t. This was where the pain was coming from when the patient arrived at the clinic following a trip up the stairs. </p>

<p>The reference to the word &#8216;effusion&#8217; within the image means swelling and all the black within the groove is essentially swollen fluid that has collected as a result of trauma. What is interesting to mention with this case is there is no arthritis associated with the joint, it&#8217;s perfectly healthy, apart from the injury described.</p>



<p><a href="https://www.sussexfootcentre.co.uk/diagnostic-ultrasound/">Ultrasound</a> was able to show the patient just how swollen their toe was and why it was feeling so sore when it was bent. The swelling made for a hugely stiff joint. This was settled down with an insole to stop the joint being used and took away 60% of the pain, with a steroid injection used thereafter to remove the rest. The patient was back to running in no time!</p>

<p><span style="font-weight: 400;">Heel pain is a complex but common problem seen in the clinic every day. Although there are multiple conditions that can effect the heel the most common is plantar fasciitis. The condition doesn’t discriminate between age, gender or activity levels and can be debilitating as a result. </span></p>
<p><span style="font-weight: 400;">The traditional thought process was that <a href="https://www.sussexfootcentre.co.uk/conditions-treated/chronic-plantar-heel-pain/" target="_blank" rel="noopener noreferrer">plantar fasciitis</a> was an inflammatory one, however, now the consensus is that it&#8217;s caused by progressive, repetitive micro-tears and therefore degeneration as a result of overload.  </span></p>
<p><span style="font-weight: 400;">Before any treatment options take place we suggest carrying out an <a href="https://www.sussexfootcentre.co.uk/diagnostic-ultrasound/">ultrasound scan</a> to ascertain just what exactly is taking place in the heel. Treatment options are separated out into either conservative with the use of orthotics to redistribute pressure or when that has failed, with <a href="https://www.sussexfootcentre.co.uk/services/podiatry/cortisone-injections/">cortisone</a>. Cortisone can be considered if the pain lasts for greater than 4 weeks and the pain has not been reduced by orthotics. </span></p>
<p><span style="font-weight: 400;">Cortisone injections have a bad name as they can cause pain, however, at <a href="https://www.sussexfootcentre.co.uk/" target="_blank" rel="noopener noreferrer">Sussex Foot Centre</a> we anaesthetise the foot, which makes the whole process far more comfortable. </span>Many websites will describe stretching, rolling a cold water bottle under the arch, ice and ibuprofen gel, however, we don’t find these treatments to be effective.</p>

<h4>Case Study</h4>
<p>Patient A is a 52-year-old female who works in a fast-paced stock room so is continually on her feet. She began feeling pain in the heel of her right foot in December 2017 but put it down to muscle soreness from walking.</p>
<p>The pain continued as she tried treating it with recommended exercises and sports massage. So it was time for a visit to <a href="https://www.sussexfootcentre.co.uk/" target="_blank" rel="noopener noreferrer">Sussex Foot Centre</a>! An <a href="/diagnostic-ultrasound/" target="_blank" rel="noopener noreferrer">ultrasound</a> scan showed the extent of Patient A&#8217;s chronic Plantar Fasciitis.</p>
<p>To begin treatment, we made a custom insole which she wore for a gradually increasing time each day. Fast forward to August 2018, after finding the insoles weren&#8217;t working for her, we discussed the option of a <a href="https://www.sussexfootcentre.co.uk/services/podiatry/cortisone-injections/" target="_blank" rel="noopener noreferrer">Cortisone Injection</a>.</p>

<p>Patient B got in touch and came to the <a href="https://www.sussexfootcentre.co.uk/" target="_blank" rel="noopener noreferrer">Sussex Foot Centre</a> clinic for an opinion regarding pain in the back of his heel.</p>
<p>The pain had been present for the last 10 days and had been described as the sensation of a sudden pull and a feeling of a ‘gunshot’ going off in the leg. The patient was only able to walk wearing walking boots and even then the pain came on after only 200 yards.</p>
<p>When examining the patient he had a large bulge at the back of the heel where the pain was focal. <a href="https://www.sussexfootcentre.co.uk/diagnostic-ultrasound/" target="_blank" rel="noopener noreferrer">Ultrasound</a> was used as this is the best imaging modality to assess the back of the leg. MRI is widely regarded as the best imaging technique for looking at damaged soft tissue, but interestingly this isn’t the case when looking at the Achilles’ tendon.</p>
<p>Ultrasound can be used to look at the Achilles when it’s relaxed and when it’s stretched out. It can be used most specifically in this case, for visualisation of where the Achilles’ tendon inserts into the bone.</p>

<p>It’s important to see the healthy tendon has good clear straight lines, this is collagen and this is what a tendon should look like as it inserts into the heel bone.</p>
<p>This one, however, is what he came with. This is a chronic condition called Achilles insterional calcific tendinitis. The exact cause is unknown, however, essentially overuse and traction with inflammation on the insertion of the tendon onto the bone is the common cause. The tendon is intact on the right side albeit there will have been ‘some’ fibres that will have been pulled off.</p>
<p>The patient doesn’t want surgery for an Achilles debridement, and doesn’t want to wear a walker boot so I’ve manufactured orthotics for both shoes to take the strain off the Achilles’ tendon and prevent it from pulling where the Achilles inserts into the heel bone.</p>
<p>I have reviewed this pain already after a week and it feels significantly better. It’s a <a href="https://www.sussexfootcentre.co.uk/conditions-treated/">chronic condition</a> that will be managed and we will get the patient back to exercise in 6 weeks, but only with the strain off the heel.</p>
